# GSW On the head

Protect head wounds, monitor neurological status, and transport urgently.

### Immediate actions

1. Complete the primary assessment (ABCs).
2. Expose the head and locate the wound.
3. Check for entry and exit wounds.

### Protect the wound

Do not apply direct pressure if bone fragments or brain tissue are visible.

Lightly cover the wound with a sterile dressing.

If no bone or brain tissue is exposed, apply gentle pressure around the wound.

### Monitor neurological status

Monitor consciousness, pupils, breathing, and any change in condition.

If the patient is unconscious, maintain their airway. Keep the head and neck as still as possible.

### Transport

Transport as soon as possible.
